Well, it's literally the thing that covers the pages inside, a single sheet of card (or cloth and heavier cardboard for a hardcover) that wraps around the front, spine, and back. Although in practice, people just say "front cover," "back cover," and "spine," so I can see how one could think of the spine and the cover as separate things.
