Without Janeway and Chakotay on the ship Tuvok is captain and Tom is first officer

Whatever Chakotay's right rank - although I think he was a Lt Commander (cf Jeri Taylor's Mosaic) -, in resigning his commission to join the Maquis, he made a complete break with his past of Starfleet officer, losing de facto, the right to claim his former rank now and later. However, he continued to be called by it because it symbolically standed for experience and authority in the organizational structure of the Maquis.

It's just a theory but,
it seemed to me that Lt Commander was a an offiicial rank while Commander was rather a title, which wasn't necessary official but, gave to the person concerned, a kind of precedence => a commander would be the 1st of all Lt Com present, wheter on board or on dry land AND it would be the title associated to the position of XO too.

And Tuvok, as the new Capitain of Voyager if Janeway was definitively out, would be the most logical choice. He is the oldest Starfleet senior officer serving on board and his legitimacy (authority & experience) is unanimously recognized by all, that they are Starfleet (more 2/3 of the whole crew) or Maquis (a little 1/3). Plus, he is officially ranked Lt Commander (and after Hunters, entitled Commander), contrary to Chakotay, who was granted by Janeway, the provisional rank of Lt Comm. (after to have offically lost it by resigning his commission to join the Maquis) and became Voyager's second-in-command for only the time of their "little" escapade in the DQ.
So, if events had to turn badly and Janeway was in incapcity to hold her role of Captain, the wisdom would command Chakotay to propose to Tuvok, at the same time, the seat of captain* (because of his legitimacy & his experience and respect he gets from everyone) and his services as XO (he seems to appreciate this role). And Tuvok, to accept the double offer, because it would unquestionably, the better for everyone aboard => the change in the continuity, I guess. :).
But a thing is almost absolutely sure, the relationships between himself and Tuvok would be very formal but not less conflictual! -> contrary to Janeway, Tuvok will be guided by the logic and not by the emotion).

(* I don't think that Chakotay would take the risk of refreshing an old conflict (that he'd certainly lose quickly or on the duration, besides) between Starfleet and Maquis, in trying .a coup de force to prevail himself as the new Cpt).

About Tom Paris, as First Officer... Nope. he wouldn't be interested and I don't think that he'd be too happy not to be associated with too much responsibility. His main goal remains to enjoy life in doing what he loves (piloting & making holoprogram), while being a good man (husband and father-to-be, friend). However, he could be a good (and unconventional) XO and even Cpt...

Tuvok leapfrogging over Chakotay to the Captain's chair seems like a good way to start a race war.

In Before and After, Captain Chakotay seemed fine.

As far as the costume department in the real world is concerned (I said this months ago) they never made a provisional rank badge for a Full commander.

It does not exist.

That could simply be an error.
I happen to think that Chakotay is a Lt. Cmdr, and Janeway herself is only a Cmdr, but with a brevet rank of Captain as she is the ships "Commanding Officer"
Her paycheck should say "COMMANDER JANEWAY, C.O. USS VOYAGER"

No money, no pay-check. ;)

Custom made Jewellery is expensive.

Yes, it was a mistake.

Too costly to remedy, by making a replacement rank badge listing the man as a full commander.
