IIRC one of the web episodes cleaned up this problem by placing a nurse still in the hospital for quite sometime after the world had gone to shit and the ZA?

I wonder if they did this to lock in actors pay in advance of season 5 results? If they do as well in season 5 as they did in 4 - no doubt several people would be asking for a hefty pay raise.
At the moment, I think the only actors that can get away with asking for pay raise is the actors that play Rick and Daryl, with the first being the lead and the second being the breakout character of the series. I also think Carl is safe from death, but I don't think that actor is paid all that much since he is still a minor.

I imagine anyone that asks for too much money the writers will kill off, so the actors won't be too loud about it. One of the advantages of a show like this with a high morality rate. Only five actors from the first season are still around, the rest are dead.

I think the morality rate is pretty low, with all that they've done. And the morale-ity rate might just be picking up, with Rick's quick speech.
But the mortality rate-- great point. Now, if they really want to shake things up, if they kill Rick, then audiences will be slapped with the fact that no one is safe at that point, and Darryl fans will be freaking out.
And if Rick dies, that could open it up for Carl to move on with another group (since he's getting too old to play a boy; killing him off wouldn't be necessary, and not predictable).

That's hard to buy into. One can allow for a lot for Rick's survival on the hospital bed without nutrition or water while in an unpowered hospital room in the south in the late summer.
I'd be willing to grant a week or two, and that's really stretching things when it comes to dehydration and what the body has to resort to for nutrition without any incoming calories.
But two months? Way too long for Rick to be lying on that bed.

Don't we get the sense in the flashback, that Lori is at least considering bedding Shane, if not already doing it before the ZA? Yea. There are several things about the early timeline that make zero sense. Not only the length of time he was in the hospital, how fast the world went to shit, but also notably, how quickly Lori fell into bed with Shane after the ZA and Ricks presumed death. Sure didn't take her long if you even believe 3 weeks or so.
Then again - Carl's quick recovery from a nearly fatal gunshot wound on the farm doesn't make sense either.

I was going more with the Flashback(s) showing Lori/Rick having issues, and the flashback(s) of Shane and Rick. Between those two, sides, plus Shane telling Lori about Rick getting shot (As well as how soon they were taking care of each other's needs in the first camp) I got the impression Shane and Lori were already hiding a developing relationship when the ZA hit.I don't remember any pre-ZA flashbacks involving Lori and Shane.

As noted there was one when Shane tells her Rick was shot and is in the hospital, there's also another one (though not strictly pre-ZA) of Shane and Lori on a highway outside Atlanta once it seems like things reach a breaking point, Atlanta is bombed, and they meat up with Carol and her family. Possibly the beginnings of the quarry camp.
