"Caretaker"-Will a special guest appear? Possible spoilers.

The Doctor's been through 12 regenerations and close to 2 thousand years older by now. Would he even remember Ian and Barbara?
Um, yes? Unless he'd forgotten about Susan, so then maybe yeah... but how likely is it? Very unlikely.

For the sake of argument, would a parent necessarily recognise one of their children's childhood friends many,many years later if they hadn't seen them at all in the interim?

No... as evidenced by one of my infant school (i.e. age 4-7) classmates turning up as a newly-qualified teacher at the school my Mum teaches at 25 years later. ;)
 
If he remembered Clara after 800 years (bar a brief meeting 300 years in) then remembering Ian wouldn't be too much of a stretch.

But she was the last companion he'd had before settling down on Trenzalore. In essence, she was the last Earthling he'd seen. Ian was nearly a millennium earlier.

I've no strong views on it, a story where he recognises Ian right away is fine by me, as is one where it takes him a while to place him.

I don't think that's quite the right analogy. Ian and Barbara were already adults when the Doctor met them. Someone who ran around as an adult would be more recognizable than someone who was around you when they were children, wouldn't you say? They'd already grown into their skin, after all.
 
^But from the perspective of the Doctor, a being whose lives are measured in the centuries, a young man in his 20s might as well be a child. Plus an Ian in his 20s and one in his 70s or 80s would have changed radically in appearance.

Think of how old Peter was when Jo Grant was gracing the telly screen.

Then think of the infamous Katy Manning photo shoot with a Dalek.

Guess who's getting a wish fulfilled. Right there.

Aren't they filming the Christmas special right now? Jo showing up there would be a real treat, especially if Clara's moving on by or before that point. Maybe even as a minisode made at the same time...

And sorta back on topic, there was an outside but not specific reference to Ian & Barbara, but that's all we got. More's the better. :)

D'oh, a swing and a miss with even a specific reference for Ian!

As for Katy, she's a real sweetheart. I had the pleasure of meeting her at the 50th anniversary celebrations. There was surprisingly not much of a line to see her so we chatted for quite some time. In the middle of our conversation she caressed my cheek. Seeing what a friendly, bubbly lady she is, I'm sure that's pretty normal. Would be great to see her back in Doctor Who!

The only thing they missed was the angle of the set behind them. Of course, this means that in the 2014 edition they're arguably pointing AT something besides the round things on the opposite wall. :)

Also, is that a clip of some sort pinning Capaldi's coat open?

^ How does that indicate someone is coming back? That link is about the Doctor Who experience.
 
LIkewise, Janet Fielding was spotted visiting the set a few months back; there was a lot of speculation on that too, and but I doubt the "Gobby Australian" (sic) is coming back either...
