What's to say the Q haven't done everything everyone's suggesting in some other universe out there? In the Trek one, they've decided to play with the nature of things as is. They won't give everyone a flying car and immortality (as they did in universe # 9687687667878) but they will nudge and judge here and there.
Maybe they leave people to suffer for variety's sake or because they're exploring how the universe unfolds by itself, but they could have intervened in infinite universes for good and in infinite others for evil and infinite others for shits and giggles and have there still be infinite others in which they just haven't gotten to - that's the nature of infinity for you.
