How would anyone know? Assuming Starfleet has a "don't ask, don't tell" underwear policy.

In Kirk's time, Starfleet had a "bloomers only" rule. Examples can be seen in almost any TOS scene featuring Lieutenant Uhura. Later, the rule was dropped and personnel were allowed to choose any type of undergarment they preferred.
This led to the unexpected but wildly popular arrival on the scene of the so-called "Starfleet thong."
Unfortunately, due the the reactions of a bunch of stodgy admirals back at Starfleet Command, it also led shortly thereafter to the demise of the TOS mini-skirt as the default female duty uniform.
I can't help but wonder what Helen Noel's air duct scenes would have been like with a Starfleet thong...

Don't those under-the-jacket turtlenecks snap under the crotch? I swear I heard someone mention that at a con. Or it came from a horrible part of my subconscious. Although I guess they didn't wear those type of uniforms on the ENT-D most of the time.
Maybe they don't need underpants, if that's the case.

I was under the impression that undies were built into the uniforms
for instance in Chain of Command when the spoonheads cut Picards 'sneak suit' off he appears to be totally naked, and it didn't seem like they cut his suit off then cut his space-boxers off . . . just his suit

Here we go again! The skant (which was a divided skirt, more like culottes) was a unisex alternative casual Starfleet uniform, worn by several male and female extras, plus Troi plus, in her final scene, Yar.
It's true for some of the undershirts. But that was only to keep the uniforms looking straight for continuity during filming.
For golf on the holodeck though, they always wore two pairs of underpants each. In case they got a hole in one.
The skant is cool. There should be more skants. I would LOVE to see Tuvok in one.

Harry is definitely skant material! It looked like he might get a bit skanty (hmm..) in Favorite Son but I don't think we ever saw his legs.
