What holodeck program would you create?

No. I will certainly use the LOTR scenario. And the latter point is impossible.

I would probably re-create a really cool dance club and would invite the Voyager crew to a boogie night. I'd pick the songs to dance to and it would be one hot night of dancing!!!!

I love to dance, can I get an invite? I also liked the luau program and would have loved to play velocity. That looked like a great game!

I am not sure how that would work though. It would be essentially be an animated board game. Not that Civilization is not good.

As much as I love Civ (I have 2, 3 and 4 installed on my main computer) I don't see that it would benefit much from the holodeck. Better to play it on a computer-thingy in my quarters.

I'd use my holodeck time for action-adventure (Doom or whatever) if I had sufficient faith in the safeties. And also for the reason holodecks were invented in the first place.
