There was no cancer risk.

In the 22nd century it was discovered that if rats aren't killed by predators, starvation, or other environmental factors, they all get cancer at a certain age and die. It invalidated more than a century's worth of research that showed that everything from cigarettes to water caused cancer in lab rats.

Protomatter enabled the Genesis Device to work on a planetary scale.
Seemed like the cave was small scale and more Carol's work, while David took responsibility for the big planet-shaper.
David was fairly young and might not have even been a part of the earlier work. My personal explanation is that after joining the project, David discovered that his mother's theory simple could not be made to work on a planetary scale. Knowing how important the project was to her (her life's work) he and some of the other young researchers inserted a sub-program to create proto-matter into the Genesis matrix.

Given the size of the code, Carol never saw it.

David Marcus sure had a lot to answer for. And he paid for it with his life in STIII. A shame as he was an interesting character and could have gone far with some nicely written moments between him and Kirk beyond what we got with TWOK.

There is no direct evidence on screen that the Genesis project was a failure.

1.>The Genesis cave appeared to have been stable for some time.

2.>The Genesis Device was never properaly tested. It was designed to terraform an existing lifeless planet into a Class M world. Not create one.

3.>Circa 90 years later protomatter was used succesfully to re-ignite a dead star, Epsilon 119.

Noting also that Dr Carol Marcus did not show any concern whatsoever about Kirk's choice of food.

Now, it COULD be that she secretly wanted to see Kirk suffer and/or was using him as a guinea pig, but personally I doubt it. She came across as being very principled, not the sort to do that sort of thing even as a joke. I think it more likely that she was sufficiently confident about products of the Genesis Cave that it was no longer a big deal.

Obviously he never got it, remember Treks III-VII? If they were going that route they would have put that in somewhere in those films.

If it is applied correctly to a lifeless planet, it will not be a problem because the device does not create a planet and star, just a self-sustaining biosphere.

Who says that the apple Kirk ate was a product of the Genesis experiment? It could have been that Carol had stocked the cave with enough supplies to "last a lifetime" in case they had evacuate the station and hide inside Regulus.
Missed this before. But when Carol says there's "food in the Genesis cave" and when McCoy says, "We thought this was Genesis, she dismisses the idea. She clearly means the Genesis cave, and she clearly means the food comes from it. As per the script (emphasis mine)...

Kirk and the others eat fruit from the huge (matte) forest beyond. Chekov has been moved to join them.

As an aside, they actually shot that scene; it's in the incomplete work print in the Film & Television Archive at UCLA. The line readings make the spotty dialogue even worse, though. It's no wonder it was cut from the finished film.
