Transporter

I grew up watching Star Trek. The transporter has been used in a few episodes from traveling to a parallel universe to combining two individuals into one and back again. To convert energy to matter could be very dangerous. Both fusion and fission convert only small amounts of matter into energy with very destructive force. I would think a device that can turn matter to energy and back again would be a dangerous device if it malfunctioned. I was curious if anyone knows of anywhere in the Star Trek universe this has been a problem?

Fusion and fission release energy, and in the case of fusion sometime there in a large absorption of energy instead.

Given the frequent use of the term "matter stream," I think Kirk "short handed" use of convert to energy when speaking the Lincoln wasn't completely accurate.
