The Ceti eels didn't actually enter through the normal ear canals. You'll notice they have to punch holes in Chekov and Terrell's ears. So if they can do that, they can get in through anywhere.
Although an eel that enters through, say, the leg, would probably be destroyed by the body's immune system before it reaches the brain.
Perhaps the eels can punch through pretty much anything humans could use to cover their ears?
