She saw Kate with Aaron and talked to her. She knows that Claire was on the island and was pregnant and Aaron might look like she did at that age. A mother could put 2 and 2 together...

I think she realized that there was no way Kate was that far along if she was pregnant at all. My hesitation came from whether she had the funds for all this since Christian paid her hospital bill.
