For what it's worth, the novel version of TWOK has Khan saying that eventually, the victims of the eel succumb to paralysis, unable to feel anything but the eel squirming inside the skull (man, that's messed up, lol). So that wouldn't jive with the eel coming out of Chekov's ear when it did being part of the full process. Obviously, it was forced out somehow.
