Actually, the audience is making an assumption as to McCoy's throwaway line being the reason for the nickname. It's a good assumption, but still just an assumption.
Kirk could have easily forgotten that reference, and the nickname could have originated later (or earlier) by any number of sources.
And as for the original timeline, we still have no canon explanation for the nickname, so make up whatever you like.

To me, that was their answer to the nickname, that's why it does done on purpose.
I am going to stick with that opinion for now.

Well, again, I don't see how it was their "answer" to the nickname as it doesn't really seem like they infered as much in a throw away line that seemed more a wink to the audience than anything else.
I had heard too, way, way back that Bones was short for sawbones. I think I've just always assumed that was the etymology of the nickname. The alternate version doesn't bother me, but now that you mention it, it does kind of come across as a bit insensitive. It gives new meaning to the line: "You of all people should know the dangers of reopening old wounds, BONES!!!"

I think it rings even more true for that reason.

All right. But not everything has to have a proper reason or meaning behind it; perhaps the nickname "Bones" is purely because Kirk likes it, for some reason or the other.

Well, his real original nickname was "Old Dirty Groin Groper", but they made him change it to Fuckface.

Not only does such a genesis for a nickname make no sense, "Hey when we first met you said your wife left you with your bones after the divorce. It was an off-the-cuff flippant remark and using it as a nickname will likely stir up painful memories of your divorce!"
