I know a few crew members got wacked during the 3 seasons, but the last officer I can think of is Ensign Rizzo in Obsession.
If I put that particular question in one of the other series forums, it could give it away. I wanted to provide a better challenge than my previous question.
However, I can give the answer and move on to another question if it would be more appropriate to use this for TOS questions only.
