Isn't it strange that there are so many lieutenants in a security team of two or more members? In TAS (Sign of Lorelei) there is even a team of female security led by Uhura consisting of 4 lieutenants!

Maybe ensigns were considered too green for the situations that those teams were dealing with.
On the reverse, I think a lot of officers top out at Lieutenant, or more likely Lieutenant J.G. as those first couple of commissioned-rank promotions come pretty much automatically just from consistently doing a good job, but going into the Command ranks was a whole 'nother ballgame. Even full lieutenants take on more leadership responsibilities as department heads/chiefs and so on.

That effect is just as amazing as ever
Jay Jones' story about this stunt is amazing. The timing of the explosion was off and he took the full force right in the chest. He had dirt impacted in his sinuses and couldn't breathe and, in his words "I was like a human fireball." According to Jones, there was another angle filmed that was too violent to air. He wound up in the hospital. However much is accurate, the final scene makes a damned convincing argument for it.
