Isn't it strange that there are so many lieutenants in a security team of two or more members? In TAS (Sign of Lorelei) there is even a team of female security led by Uhura consisting of 4 lieutenants!
 
Maybe ensigns were considered too green for the situations that those teams were dealing with.

On the reverse, I think a lot of officers top out at Lieutenant, or more likely Lieutenant J.G. as those first couple of commissioned-rank promotions come pretty much automatically just from consistently doing a good job, but going into the Command ranks was a whole 'nother ballgame. Even full lieutenants take on more leadership responsibilities as department heads/chiefs and so on.

Well, redshirts couldn't die on Saturday morning television, so all the ensigns assigned to security who would've been killed in the line of duty in live action Trek survived to be promoted to the lieutenant grades.

Jay Jones' story about this stunt is amazing. The timing of the explosion was off and he took the full force right in the chest. He had dirt impacted in his sinuses and couldn't breathe and, in his words "I was like a human fireball." According to Jones, there was another angle filmed that was too violent to air. He wound up in the hospital. He and his dad watched the episode when it aired and his dad, a veteran stuntman himself "felt ill watching that, he knew I really ate it."

However much is accurate, the final scene makes a damned convincing argument for it.
