Smith is, basically, the 13th, as the Meta-Crisis is the 12th. I think. Or, if you don't count the Meta-Crisis, the 12th Doctor is basically the 13th Doctor.

Moffat does count the meta-crisis, or rather the amount of regeneration energy used to create him, which was almost equal to a full regeneration even though it was aborted at the last second.

Capaldi is the thirteenth physical incarnation of the man, but the twelfth to call himself "The Doctor". Tennant was only the eleventh incarnation, but wasted one of his potential lives by not completing a regeneration.

Well there is "The Curse of Fatal Death" method. Have a blink and you'll miss him 13th Doctor which regenerates into the 14th Doctor the same episode as the 12th regenerates into the 13th. Still Moffat.

Would add troll casting in there just to mess with people.

It's surprising how much of that spoof has informed Moffat's era proper; the Doctor getting married, the timey-wimey shenanigans, the "I'll explain later"s... the only thing left is, "Clara, look; I've got etheric beam locators!"

To troll, the 13th Doctor is played by Jenna Coleman. She regenerates the following episode into another form. (The Doctor has said that sometimes it goes wrong, and we've seen Romana able to change her form several times before settling in on a new face).

Clara is still the companion just to make it messy.
