Odd question, debate (for fun)

Can you get your toaster pregnant or catch an STD from your hoover? No. Same applies to holograms. They're tools, nothing more than that.
Can a toaster be sentient? Meaning, they're very different kinds of things... your point is better in that, like a toaster a hologram is not biological... no biology no procreation.
But then that kind of logic falls apart (in universe) I we use a parallel of your example: Can you get a monkey pregnant? A sheep? No. Same should apply to aliens, they're different species (however similar in appearance).

Well, a holo-virus (holo-flu for example) would interact with normal matter, I suppose. I mean, the holodeck can let its holo"matter" interact with real matter on a macro scale, so why not on micro-scale ? So a holo-virus would enter your cells, and replicate using real matter that already was in the cell, and the next generation that would come out would be 'real' flu viruses. So if you get infected with a holovirus, I suppose you are infected for real.

However, holodeck bacteria in your body would simply disappear once you step off the holodeck ?

Macro and micro are very different... but either way... Holograms don't interact with matter, per se. They are photons manipulated by force fields. So then, how do force fields interact the matter? Apparently electromagnetically to create physical forces. This is more akin to mechanical process. So how many biological processes could be performed mechanically (because essentially they are?).

Neelix's lungs (assuming human like operation) only needed to filter out/extract waste and CO2 and add oxygen. This can be done mechanically and using a little basic chemistry (ECMO https://en.wikipedia.org/wiki/Extracorporeal_membrane_oxygenation). Any complex biochemical processes that might also need to occur that can't be mechanically replicated would fail... this could be the limitations (as they are in modern technology) of such an apparatus. Barring cloning/live transplant, they didn't have anything to replace his lungs. I believe even then they couldn't replicate live matter... so even if they had been able to replicate lungs, all the cells would've been dead, making the lungs useless.

You do bring up a good point... can force fields (in-universe) be used to manipulate micro-matter? Neelix's lungs say yes. But assumes virus' work like nano-bots, building copies of themselves as opposed to reproducing via mitosis/binary fission, but if they did? The nano-holovirus is actually a robot that builds "real" versions of an actual virus... synthetic microoganism? Yup, that would work... Not the same as actually being infected by a holovirus though.
 
As a fan of SF&F, I'm ashamed to say I've watched hardly any NuBSG, so I don't understand these references.

Meh, I'm a huge SF/F fan... never watch BSG or NuBSG... not ashamed... seen a little here and there, never really grabbed me... oh well.

OOH: DS9:Babel, a synthetic virus is built at the molecular level... how this is different than replicating simple life, idk. But given that, it's possible to get STDs from the holodeck (on DS9... maybe it was the cardassian replicators)... STAY OUT OF QUARKS HOLODECKS!!! (you absolutely know they're filthy with HVDs (HSTDs?).

But any damage to your body would remain yes? Even after the cause was gone.

And your body might continue to have coughing, sneezing, fever, inflammation, vomiting, diarrhea, fatigue, cramping until your body figured out that the bacteria was gone.
