Ms Word Image- Create Text Boxes

I have an image of a form that I scanned and placed in MS Word

I would like to be able to create text boxes in the doc that I can type in making it a template.

How can I do this?
 
Simply draw a text boxes where you want them, making sure they are in front of the image. You may want to get rid of the border for aesthetic reasons. You might also want to adjust the layout properties to get the boxes where you want them (format text box->Layout tab->Advanced button).

This seems fairly trivial, perhaps I'm not understanding exactly what you are asking.
 
Oh, you want to add the text boxes to the image itself, eh? I don't think you can do that exactly. The best you can do is make the text boxes and group them with the image. However, if you're using Word 2007, that's trickier than it was in older versions. After a few headaches caused by this, I found this explanation. The short explanation is that the new graphics engine in Office 2007 isn't fully implemented in Word so some different types of objects can't be grouped. One solution is to save the document as the older .doc file type, which then makes Word treat everything the same and they can then be grouped.
