The only Doctor to not have been cast prior to his predecessor's regeneration was Pertwee, and that's why he wasn't in The War Games. Everyone else was publicly announced well before the regeneration episodes aired. Yeah, I distinctly remember hearing the casting announcements of both Peter Davison and Colin Baker before they made their debuts (I was too young for the earlier ones and had lost interest by the time of McCoy).
