Indeed it is from Tuvok's mind in said ep. IIRC isn't it from the movie Event Horizon? I know one of the images they used is. Another is the planet exploding from ST: Generations too

Hell yes. Nearly every episode had that exact same shot. But going out on a limb and i'll guess "Cold Front"?

Nope, sorry. Notice that they're at warp, it's a signifigant story point for the episode. (TV being so full to the brimming with cheapness they would unless the story specifically called for it just have the impulse background)
