What they should do would be to keep things subtle (start resisting what the Federation was doing without openly breaking the alliance). Since this is Toral we're talking about, he probably would have done something stupid like declare war on the Federation.
How long the Duras family stayed in power would be hard to say, though, since any failure on their part would lead to questioning of their rule (especially if news of their Romulan connection was made public). I'd imagine the Romulans would have to spend twice as much effort keeping them in power as they did in helping that gain control.

Wouldn't all possible Klingon governments be subject to such scrutiny? Any Chancellor or Emperor probably has to keep constant vigil against his enemies and friends.
Yet there must be some sort of safeguards to protect the Great Leader from constant dueling challenges. If such safeguards could keep the fat K'Mpec in power, they could keep the young Toral there, too.
That is, as far as dueling is concerned. In terms of politics, it seems that powerful Houses working their influence in the shadows are the dominant force, or were in K'Mpec's time. What is done on the open fori is less important. And if the Sisters take care of the negotiations with the Houses, Toral can probably blurt out the most idiotic things at the Great Hall or in the Council Chambers and get away with it.
Still, the boy will probably soon become a liability for the Sisters. Unless he grows up to be another Alexander. (The Great, I mean, not Rodzhenko!)

We saw how quickly support for the Duras fell away when the Romulan connection was exposed. I don't think the council members who supported the Duras would tolerate them if they learned that they were being manipulated by their most hated enemy.
The Duras family would have to ensure that Worf and Kurn were dead for a start. Neither would stand by and allow a family of traitors to rule the Empire.
