How many times has an actor/actress done a guest spot *on screen* and then later joined the series - as a new character? Here's the ones I could think of ...

Lala Ward - was Astra, then became Romana 2.
Colin Baker - was Maxil, then became the Sixth Doctor.
Freema - was a Torchwood agent, then became that character's cousin, Martha.
Karen Gillan - was one of the soothsayers, then became Amy Pond.

Special nomination for Nick Courtney who was Bret Vyon, then became the Brig.

Jacqueline Hill would not count for this. She was a regular first (Barbara) before coming in as a guest, Lexa.

Are there any I've missed?
 
Eve Myles played the psychic girl in "The Unquiet Dead" and later became Gwen Cooper on Torchwood.
 
Bernard Horsfall was one of the Time Lord judges in "The War Games" then played Chancellor Goth in "The Deadly Assassin." Although those could have actually been the same character (the first one was never named).
 
Peter Purvis who managed to star as two different people in the same serial (he's the cowboy atop the empire state building in the Chase, then obviously comes back as Steven)

Right, just like with Freema ... coming back to play a look-alike relative. How did I forget her?

Oh that's right, I can't believe I forgot about that. He just kind of showed up at the end "Hi, I'm here. Let's go!" lol
 
Clive Swift played Jobel in Revelation of the Daleks and Mr. Copper in Voyage of the Damned.

Richard Briers played The Great Architect in Paradise Towers and appeared in a cameo role as a dying recluse in Torchwood's A Day in the Death.

Also, and a bit less obvious William Hughes played The Master as a child in The Sound of Drums/Last of the Timelords and also appeared as a young boy in Torchwood's Sleeper. I don't think either of these roles were speaking roles though.

I assume you're just talking about people who later became regulars, rather than guest actors who just reappeared later as a different character. (Although some posters seem to have interpreted it that way. In which case, we'll be here all day and have to mention pretty much every actor working in Britain in the sixties and seventies!)

Anyway: Ian Marter played Andrews in "Carnival of Monsters" and later returned to play Harry Sullivan.

John Levene was a yeti in "The Web of Fear" before becoming Sergeant Benton.
 
Nicholas Courtney was in The Daleks' Master Plan then went on to be the Brig.
