Did we ever find out

1. Who blew up the TARDIS
2. Who or what the mysterious voice was that said "silence will fall" on the TARDIS monitor screen

3. Who got Kovarian into hunting the Doctor
 
In no special order, the Silence destroyed the TARDIS, they and Kavorian broke away from the church under the impression that the Doctor was piloting the TARDIS when it exploded. And no, we never did find out who's voice it was that River heard in the TARDIS.

But the silence alone couldn't destroy the TARDIS. They had to tell someone to do it. I suspect it was River as she was the one trapped onboard. So she died twice.. Once there and once in the Library..
 
1. The Kovarian Chapter, as revealed by Tasha Lem in 'The Time of the Doctor'. They blew up the TARDIS, creating the very crack(s) in time through which the Time Lords were using to try and break through into their own universe. The "Destiny Trap", as the Doctor described it. The mechanics of how they did it were left unrevealed.

2. No, but I think we can infer that it was a member of the Kovarian Chapter. I suspect it was intended to be the voice of a Silent, but they changed the voice between seasons to make them sound a bit scarier?

3. Kovarian was apparently "frustrated by the centuries-long stalemate" at Trenzalore. She was just the leader of the splinter group that split from the Church to try and stop the Doctor from reaching Trenzalore in the first place. If it hadn't been her, it probably would have been someone else.
 
Orla Brady and Frances Barber both appeared in Hustle as conned art collector and fashioned designer. They didn't like losing, so they left Earth to form the Papal Mainframe. I guess being Walter Bishop's wife didn't help. It takes a toll on a person. So she told him “I am leaving. Take care of our son Sherlock. We are doing this crossover... The long way around...”


Oh, where was I going with this... Yeah, so basically Watson, er, I mean Bilbo Baggins, blew up the TARDIS.

When the Kovarian Chapter breaks away from Trenzalore, they don't just go shooting at the TARDIS until it explodes. The Doctor wouldn't be stopped that way. No, they set up the Silence on Earth. They set the stage at Demon's Run to abduct Amy and her baby. Eventually that baby grows up to be Mels, who tries to kill the Doctor.

I like to think that River still had some subconscious "kill Doctor" things in her brain. It's very possible that she attempted to destroy the TARDIS in "Pandorica/Big Bang" without even knowing she did it.

And doesn't that mean she had 2 on screen deaths if that's true? one there, and one in the library?
 
Well, no. The TARDIS saved her in the control room until the Doctor rescued her. She was in the explosion, but she didn't die.
 
In "The Big Bang." She's stuck in that time loop, where she keeps running to the TARDIS door saying "I'm sorry, my love" as the control room starts to explode all around her. The Doctor tracks her signal from the roof of the museum, and then he uses the time vortex manipulator to go get her.

I mean, River is in the whole rest of the episode. How did you think she got there?

I know but i missed her rescue
 
Don't forget (;)) that there was a Silent in the TARDIS in the preceding episode "The Lodger"; the implication is that it rigged her to explode at that point. Whether it was by straight up planting a bomb or by some other form of sabotage, we don't know.

And as for the voice, was it not Kovarian's? I thought it was, but maybe I'm misremembering.

There was? TARDIS Wiki makes no mention of this. They do mention a Silent's corpse being in the console room of the time ship above Craig's flat, but none in the TARDIS.

Kovarian is the popular theory that most assume to be fact now.

It was a subtle, unspoken thing. Basically Amy reacts to seeing something in the console room, but turns away to comment, but forgets and acts as if nothing happened. There are YouTube videos of it.
 
Yeah it was something that only became clear in retrospect after they were properly introduced the following series, but it's obvious when you watch it now that it was direct foreshadowing.
 
I somewhat suspect it was Prisoner Zero. The voice is similar. Also that might have been his crime. Blowing up a TARDIS. Time and the Doctor don't go in order when it comes to the Silence and Trenzalor.

I never really considered much about Prisoner Zero beyond the one episode, but I kinda like this idea. It would certainly explain why Prisoner Zero seemed so proud.

"The cracks in the skin of the universe. Don't you know where they came from?" *cracks an evil little smile* "You don't, do you?"
