Surely, he couldn't have lived all the way into his twilight years escaping into holoprograms and neurotically insecure all the time...and alone.

I'm sure that he found at least one real woman, but why would he need to hook up with a real woman? TNG and DS9 told us that holo-sex is common, enjoyable, and just as good as the real thing. Riker evens tells the bridge that he going to get some sweet holo-loving in the episode with Famke Janssen. Voyager even indicates that holo-companions are possible and, in some cases of extreme anti-social behavior, preferable alternatives to real relationships.
Barclay's problem was that he couldn't turn off the fantasy when he was finished and became addicted to the incredible sensations of power and joy. As a result of his addiction he became irresponsible in his duties. All that was required of him or any Federation citizen was to fulfill his duties and commitments. Once that was complete he was free to do whatever he wanted to do in his private life.
Honestly, if you could recreate your own Deanna Troi whenever you felt an itch wouldn't you? You know that Barclay did it, at least, once.

That's where the holodeck would be so dangerous if it really exisited.
People would be able to fufill any/all their fantasy's and reality would suddenly have too much competition. It wouldn't stand a chance against the fantasy. People would lock themselves away and the human race might be in danger of becoming extinct.

I doubt it. Look at Geordi, who's also a little nerdy but was far better adjusted than Barclay ever was. You can count his dalliances on one hand. All we know for certain is that Barclay was manipulated by a woman pretending to like him in VOY's "Inside Man", and a character tried to hook him up with his sister (and failed) in "Pathfinder"; but that's about it.
Unless one wants to speculate about that walk in the park in "The Nth Degree" he got with the goddess of empathy; that's pretty much it. Barclay was a neurotically introverted guy.

Holo-sex, yes. A real relationship would obviously be impossible. Reg, for all his social awkwardness, seems like the type who would want a real, committed relationship (his issues can probably be traced back to loneliness). A holodeck girl would be only good for sex, and I doubt Reg is that shallow.
And of course there's his past problems with holoaddiction to consider.

And, even if we did, we would certainly abuse it far more often than make it something more practical on our behalf.

I doubt I would abuse it. Replicate Teri Hatcher when she was 30, check. A good 1hr romp, check. Replicate Hang Glider scenario, check. A good 2-3hr romp, check.

We saw some of this on the show; through his fellow engineers making him realize that he's part of a team and that his abilities (which are considerable - awkward or not, he's a talented diagnostic engineer) are greatly needed. People like Reg want, more than anything, to be needed, to feel useful. If Reg realizes that he is both of those things, then he won't feel the need to retreat into the holodeck all the time.
