I've only just realised that the Colin Baker story Attack of the Cybermen contains a major continuity blunder, in that the Sixth Doctor remembers meeting Lytton when he was working for the Daleks in Resurrection of the Daleks....but they never actually encountered one another in Resurrection, so how can the Doctor possibly recognise him?

EJA means that while Lytton would know of the Doctor (as he was working for the Daleks who were setting a trap for the Doctor), the Doctor never really meets Lytton.
Lytton interrupts to stop the Daleks exterminating the Doctor (in the way that often happens just after a cliffhanger!), but otherwise they don't meet onscreen in any meaningful way, down to Lytton's name never being mentioned in the Doctor's earshot.
But there might be an offscreen scene. Or an untransmitted story (I'm sure Big Finish would have done one by now if Maurice Colbourne was still alive).
