Ro was made to remove her earring by Riker when she first came aboard... It wasn't until Ro made her "deal" with Picard that Riker had to give.

Kira wasn't starfleet

Worf's sash was no doubt another exemption granted by Picard.

Is it necessary to have a sense of humor? Yes. I'm not trying to start a holy war........just commenting on an observation. Calm down, everyone.

Worf was rather neutral about his sash, so nobody had a motivation to throw the book at him. Ro made something of an issue out of her earring, though, and was despised by Riker to begin with, so he took the opportunity to evoke the little-used regulation to put her in her place. And Tuvok was bitchy for pedagogical-disciplinary reasons, for which the rare regulation only provided an excuse. Three different ways to make use of the same regulation...
