Back when Gene Roddenberry developed the rank system for Star Trek who helped him and was the rank of Fleet Captain his idea or did someone suggest it?

Well, just like it was said the other recent thread on ranks, we're not sure if Fleet Captain ever was planned as a rank (actually, to be honest, I'm not sure how carefully Roddenberry planned the rank system, judging by the fact that Spock changes rank without changing his uniform's rank markers).
