Actually, River bought the vortex manipulator from Dorium Maldovar, who "obtained" it from a handsome Time Agent. There's no indication the Time Agent had been killed, just amputated.
I prefer to think it isn't Jack simply because that would be small universe syndrome.

Absolutely loved it, so glad I avoided anything that even hinted at being a spoiler and so Mels becoming River took me totally by surprise, it shouldn't have, hell when the sports car roared up my first thought was "River" but it caught me off guard, all the best twists are obvious in hindsight.

Actually, I took that to explain how Melody could've regenerated into toddler-Mels in 1969 New York but was still only 10 years old or so in 1996. It works for the inevitable subtle discrepancies in River's apparent age, too.

I feel like it's fallen under the radar a bit, but I absolutely loved the Doctor's brief confrontation with Hitler in this episode. The defiant and disdainful tone with which he says "It's a police telephone box. From London" was absolutely perfect.
I didn't think I could love Smith's Doctor any more than I already did.
