I have worked this out, and am pretty sure I know what it is. However, this is based on a lot of spoilers for Demons Run.

The Doctor carries in his TARDIS a cradle. He says it's his cradle. And written on the side in Old High Gallifreyan is the name 'River Song'. Or 'Melody Pond'. Or some other combination of terms conveying a meaning of water and music.

Now the Doctor has never wondered why this is written on his cradle? No, of course not. He didn't even wonder when he met a (apparently human) woman who introduced herself as River Song. Why Not?

Because as far as he is concerned the name on the cradle is his own name. Since it's his cradle.

So we can deduce that the Doctor's name in Old High Gallifreyan is some combination of terms signifying water and music.

Now, if the only water in the forest is the river - what's the only water in a cradle?

From this I deduce that the Doctor's true name rendered from Old High Gallifreyan is Piddlejingle.

There is nothing in the episode 'The Armageddon Factor' to suggest that Theta Sigma is his nickname. It's simply what Drax calls him. Terrence Dicks stated it was a nickname in the novelization, probably because he didn't like the idea of it actually being the Doctor's name.

It's like asking someone who speaks English to translate something into English.

Of course, there is the matter of why the Tardis translates other languages for the Doctor's companions, and not Gallifreyan. Personally, I suspect it's all to do with the Time Lord's secretive nature. Deliberately making sure other races can't learn too much about them.
