Sure, we won't talk about Scorpion's inappropriate Virgin Killer cover.

Here are some otehrs:

Mr. Bungle (self titled) clowns are creepy enough for many people. But this clown has a lit match and burn marks all over. And the back cover is also disturbing.

Slaughter: Stick it to ya. This might not be disturbing to some, but when the record came out, I just felt so bad for the pretty damsel strapped helplessly to the bulls-eye.

Brutal Truth, Sounds of the Animal Kingdom

Pearl Jam (self titled)

Dream Theater Live Scenes from New York (original print). It was since replaced, but I somehow found it Kmart much later.

Guns N' Roses: Appetite for Destruction (original print) The rape scene was since moved to the inner sleeve

"Nothing's Shocking" by Jane's Addiction has a pretty strange cover, a siamese twin woman sitting in a big ass rocking chair with both heads on fire...
 
Some of the ones here could certainly qualify...

I don't think Black Sabbath's Sabotage cover belongs on that list. Sure, it's not the best album cover in the world, but it's a million miles away from being as bad as the ones showcased there.
