There was a thread about Number One, but I don't know if that specific question was asked. I think they didn't use her because:
a) Secondary characters would have confused casual fans;
b) She is only known as "Number One," which was of course later used as Riker's "nickname";
c) It would have presented a problem for the plot, in terms of the chain of command (and hence, created one more person they would have to kill off.)
Of course in the Original universe, we only have "The Cage" for reference, so it's entirely possible she did serve with Pike, then went her own way.
It seems that this Enterprise was on its first mission, so no, they couldn't have served on the Enterprise before the movie.
However, I suppose it's not out of reason either to picture Pike and Spock serving together on another vessel or in some other capacity.
The Enterprise was launch before being Christen or commission or both and mostly, likely a crew haven't be assign to the Enterprise yet. When the Enterprise was launch. She was man mostly by cadets, with a few officers.

considering this takes place after the events of the cage (which in this timeline might have happened) number one, pike and spock could have all served on another ship.

I wouldn't have minded a quick throwaway line by Pike sorta like this:
"We're shorthanded, so Mister Spock will be acting as first officer for this mission. Along with several other members of my crew, my Number One was quickly given her first command and sent with the fleet to the Laurentian System..."
Only, ya know, more organic, weaved in someplace. It could have been a nice "The Cage" and TNG homage all at once.

Then you'd have more complaints about another newb getting a high-ranking post on the Enterprise.

In my "dream" version, it would have been about four years later than it was in the actual movie. Spock would have been a Lt. Cdr. and Kirk would have been a suspended Lt. (to be Lt. Cdr. upon completion of the KM) in command school. So they'd hopefully be more just a tad more plausible, at least to my thinking.
Pike's doctor in the Pilot was Dr Piper so no it doesn't match with the new Dr Perry. ( I thought Spock said Dr Puri BTW)
