I don't think this was the same Lieutenant O'Neil who went coward in The Return of The Archons since that would be after The Galileo Seven based on production order, airdate and even stardate. We need one dead guy and two injured. The dialog strongly suggests that O'Neal is the dead guy plus Immamura and Kelowitz himself are the injured ones. Also, remember the huge size of the spears used suggesting a similar death as with poor Latimer. Either that or Kelowitz gives Kirk a lousily report on the casualties...
