It was a custom of the old non-unified Empire. And yes, times change as in the 23rd century it appears the High Council had TWO female Chancellors in the 23rd century: L'Rell and Gorkon's own daughter in ST:TUC - yet by the 24th century Gowron says: "Women may not serve of the Council..."; so there's in universe precedent for the Klingons changing customs various times.Maybe Klingons in the 24th century no longer follow that custom.

I suppose there is some story potential there.L'Rell could be one of the reasons why the smooth-headed Augments became so common in the next decade and during the run of TOS. As a leader with strong and deep feelings for a Klingon who was surgically altered to look like an Earth human she might be one of the first members of the Empire's regime who felt tolerance and even acceptance towards those Klingons affected by the Augment Virus.
Those warriors had no say in their physical appearance and were simply victims of an accident of history, so why relegate them to being shunned outcasts with no power in their own Empire? Allow them to serve with honor and prove their courage in battle by facing off against the Federation and the Empire's other enemies.

The black targ of the family.Something important to recognize in light of what DSC has introduced and what it has to link up with is that one of L'Rell's chief adversaries/opponents is the House of Kor, a "namesake" member of which is himself an Augment, so that will be interesting.
