I do like this two-parter, but I am befuddled.

When we see Janeway, and later Neelix in the Klingon simulation, they've been made to look like Klingons, all bumpy heads.

But - when non-human characters, go to the WW2 simulation, they're left looking like Talaxians, vulcans and what not.

Why would the Hirogen go to the effort of making them look like Klingons in one holodeck, but not bothering to do anything to their appearance in t'other?
 
I call this different day of shooting continuity error, laziness, or it all deactivates when they leave the program.those are my theories.
 
You may notice that the Klingon bumpy heads were added in addition to their actual appearance. Neelix still had his Talaxian physical features including his ears and cranial structure, just with Klingon ridges added.

On the other hand, making aliens appear human would require removal of such natural features. It's probably easier to add extra appendages such as Klingon forehead ridges, than to take away natural portions of anatomy such as protruding ears and differently-shaped bones. In fact, the latter may well be nigh impossible.

So B'Elanna could have a holographic baby added to her person for simulated pregnancy, but she couldn't have her cranial ridges shaved down as part of the holographic simulation.
