Unintentional moments

Probably everybody knows about Layla Sarakalo's ad-libbed lines in The Voyage Home. (http://memory-alpha.wikia.com/wiki/Layla_Sarakalo)

But what about other things that just made it in the shot? Like bird sounds (see here: http://memory-alpha.wikia.com/wiki/...4th_century)#Dark_matter_nebula_planet_animal) or the fly that landed on Kirk's face in "The Paradise Syndrome"? Does anybody know of any more occurrences like this?

Whether it's an animal sound because they're filming outdoors and can't make the animals shut up, or a background performer that mugged the camera or fell...
 
Would something like the following count:

I think I recall reading somewhere that after Denise Crosby as Yar shot her last scene on the TNG, she was seen waving in the background. If I recall correctly the last scene she shot was not in the episode she was last seen in. I guess they were shot out of order. Does anyone else recall reading about this?

Symbiosis was indeed filmed after Skin of Evil, and when filming the last scene, realizing this was technically her final scene Denise Crosby does wave good-bye at the camera.
 
Crosby is way in the background, the camera is focusing on other actors who are walking through a doorway and turning up a corridor as the camera follows them. This is when Crosby waves. Blink and you'd miss it.

You mean he was smiling while every one else looked appalled? Maybe it was hysteria? However, that illustrates well that there are no small parts only small actors.
