Confronted with Kira dying from wounds from a gutless attack by some radical Bajoran religious sect, Odo realizes that while he was fine living back in the Gamma Quadrant without her so long as he knew she was alive and okay elsewhere, he can't live without her in his universe. So he forcibly demands that O'Brien assist Torres (Voyager happens to be docked, and for some reason convenient to the plot, relevant information came up earlier in casual conversation) in replicating the accident that created Tuvix so that he can join with Nerys and save her life (with her fading permission).
The resultant being can shapeshift to be Nerys when desired, and Odo when desired, and of course, all variety of other stuff and people - but all humanoid shapes including Odo's now have Bajoran nose ridges in addition to Odo's other normal "tells". The only time his tells disappear is if they are taking the shape of Nerys. As far as a name goes, they just go by Odo or Nerys as appropriate at the moment - no dumb amalgamated names for these two become one.
