Well he really didn't try to save him. Maybe it's about sacifice Gretchen did sacifice herself and I'd say teh same about HFM, but in both cases the Doctor couldn't save theme for whatever reasons.
 
My theory depends on the borg not being dead by being impaled, which is debateable. What vital organs does a thing like that have, although it probably would have "died" from that eventually. It reminds me of how warriors that were deemed worthy were taken away just before death to a paradise. But the android was creepy. I don't know. Maybe in every episode the Doctor will convince someone to die? He's done that quite a bit already, actually.

I wonder if the ones who made it to "heaven" are the ones who willingly sacrificed themselves for him (which would mean the cyborg jumped)....

Is it possible that the first soldier that was killed also 'went to heaven', but had his own, earlier & separate tea with Missy? We just did not see it?

Possible, but it depends on it's going to be everyone who dies in the Doctor's vicinity, or specific people - and it's a lot more likely to be specific people. Context is going to be everything, I suspect.
 
Watching Into the Dalek again and Gretchen's 'death' seems visually very similar to Journey's rescue at the start, which implies that heaven is, as suggested, a Tardis, and if it's a Tardis then that narrows down the possibilities of who Missy could be quite dramatically....The Master, the Rani, Romana or a brand new character (unless Missy's actually the Warlord or something!)
