Why didn't Starfleet dismantle holodecks on all starships after.....

    ......the very first major malfunction happened on TNG?

    You'd think that Starfleet would've ordered all holodecks to be dismantled indefinitely until they could make sure holograms couldn't take over the ship and try to kill everyone on it.

    After that incident, did a hologram take over the ship and try to kill everyone on it? If not, then they must have updated the software to make sure that holograms couldn't take over the ship and try to kill everyone on it.

    Well, there were also the problem of the holograms trying to kill people in the holodeck after the safeguards turned off.

    So they updated the software but allowed the safeguards to be turned on and off? Wow, very stupid of them.

    I agree, i never understood why there would be an option to turn off the safeties

    Somebody would turn off the STD safeguards too no doubt.

    I don't know. Picard was able to make the holograms become real bullets in First Contact. Who knows what else can happen with the safeguards off. :)

    Was the first malfunction incident "The Big Goodbye?" Then the next major malfunction threatening lives and the ship was Moriarty. Holographic malfunction again threatens ship and crew in "Emergence."

    If Moriarty wasn't enough, the EMH should have clued Starfleet in to the potential extreme danger of holodecks creating sentient beings capable of threatening both ship and crew.

    Why didn't Starfleet dismantle transporters after the first malfunction? A far more dangerous piece of technology.

    They just slapped a "Be warned. Enter at your own risk!" sign on the walls inside the holodeck.

    Turns out the only ships that ever had a holodeck problem were the ones we followed in the series. Weird coincidence, I know, but there it is.

    It was covered up by Big Holodeck so they could keep selling Holodecks.

    I don't know. The only answer I can give is very outdated. Back in TOS, "Court Martial" we find that the captain, the first officer and the records officer have the highest computer access.
