Who Decides to Create an Alternate Title/Theme for a TV Series?

I ran across this today and it really baffles me. Who decides whether a show needs an alternate title sequence and theme for a show sold in a different market? Is it the show's studio/production company? The syndication company that's handling the show in that country/region? Does the originating studio have a say in what's created? Does the original theme music's composer lose out on royalties for that region (probably)?

Depends on the reason for doing a different sequence, but there have been situations where the decision is made by either/or.

Yes, and often times, that's the motivating factor in making that decision. Not always, but it is often taken into consideration.
 
This is the first instance for me, of a live action series havng an alternate title sequence. I knew a few animated series did-the 90s X-Men series got 2 kick arse Japanese openings. ABC Family's Oban Star Racers was given a different theme and opening from its original French version. Even Filmation's ''Blackstar'' got a funky alternate intro for its French telecasts.
I'd just like to know the reasons why it was felt necessary.
