In terms of medical science stuff, my gf informs me House gets it right, 75-80% of the time, but obviously the drama side is not even slightly realistic.
Early ER, first say 5-6 seasons, is probably the most realistic medical drama, within the confines of a drama - i.e., it's dramatic enough for people to actually watch and enjoy it. Ergo, it has more crap than a usual emergency room. But generally, I would say it used to tow the line of realism relatively well. The really big turning point was the smallpox outbreak, imho. After that, all realism went out the window. But the Dr Greene years were always pretty good.
