Isn't it argued on and off that a planet must be unified to gain entry to the Federation? One planet, one government by the time of Enterprise.

With some countries like Ireland and Japan, the country and island names are somewhat synonymous. So unless WWIII Earth was packing some serious heat, they're still there and referred to as such.

I suspect a lot of the 'surviving' stuff is probably rebuilds - it is pretty much impossible to my eye that the Eiffel tower will survive the next 400 years just standing there, for example

I'm not sure how the Eiffel tower resisted a world war, I wouldn't expect any man made monument to be standing after WWIII.

Why not? There are lots of man made monuments that survived WWII? The only way there wouldn't be any is if there was something like complete nuclear obliteration of the entire surface. I'm pretty sure WWIII is not implied to be that bad.
