I wouldn't think a reverse regeneration would be possible. (Of course, anything is possible in sci-fi but I just don't want to open that particular Pandora's box.)
Although I prefer Tamm over Ward, if they were going to bring Romana back on the new series, I would want it to be Ward. (I can just imagine the Doctor's reunion with her, with David Tennant giving Lalla Ward a great big snog on the lips! It's not something you'd see every day and it would be one for the ages.)

If Nu Who brought back Romana it would definitely be a hot twenty-something girl. That's just the way TV is these days.
It's the same reason the next Doctor will be a twenty-something good looking bloke.
TV audiences (particularly the target Who audience) these days don't want the oldies in the lead.

That's why they need to take a page out of the original Verity Lambert/Sydney Neuman playbook and counterbalance a new old Doctor with a pair of young, attractive companions. I think it's about time that the Doctor met Ian & Barbara's grandkids!
