Can anybody tell me what's the story behind the elevator that apparently has been out of order since the show started?

It's explained as the building never gets around to repairing it.

I believe the "real world" explanation is that it gives them a chance to do exposition by having the characters walk up the stairs. As someone pointed out, it's not like the show has a lot of sets.

I would have shown up to this thread earlier, but I didn't have time to watch the episode until tonight. My boyfriend and I just finished the first season's DVDs about a week ago, so we were excited for this premiere. My boyfriend is working on his PhD in Physics, so you can imagine how much he enjoys this show.

I thought the episode was good. It's hard for me to judge because our DVR messed up and took out a chunk of the middle of the episode. We got to see it through clips on the website, but it made everything feel disjointed. But overall the episode was enjoyable.
it's available on hulu.com

I firmly believe that someday in the future, when one character says something like "miracles never happen", that the elevator will ding and the doors will open on cue.

They're saving it for the right moment. Perhaps even a final episode when Leonard and Penny move out of the building and in with each other.

I enjoyed the episode, but the laugh track was more obnoxious than usual. Even lines that weren't really jokes seemed to be getting guffaws. Maybe it was that bad last season and I just blocked it out, but I couldn't stand it!

is it a laugh track? or is it filmed in front of an audience?

It's both. Usually mixed a bit together. The 18th take is not going to get you the same laughs as the first few, so it's frequently "sweetened" ... sometimes with laughter from a previous take, but that qualifies as canned.
