SUPERNATURAL: Season six will be a season of mystery and shadow. Heaven and Hell have been left in complete disarray since the apocalyptic events of season five. And now, monsters, angels and demons roam across a lawless and chaotic landscape. And so Dean Winchester, who has retired from hunting and sworn never to return, finds himself being pulled back into his old life – pulled back by none other than Sam Winchester, who has escaped from Hell. The two reunite to beat back the rising tide of creatures and demon-spawn, but they quickly realize that neither are who they used to be, their relationship isn’t what it used to be, and that nothing is what it seems.

I wonder if Sam had spent many years in Hell like Dean did after he died and has been changed as a result of that?

Well I would think it actually (because of events they didn't plan for) would be easier for Sam then Dean (even if they spent the same amount of time).

1. That part of the pitt wasn't where others were (pretty much just Lucifer).

2. And unlike the original plan it isn't just Sam and Lucifer its Sam, Lucifer and Michael (and perhaps Jake if his soul wasn't in heaven as per their deal).

I would assume that Lucifer and Michael who have been waiting a chance to go at it, would have a nice long time to do just that. Thus the torment focused on Sam probably wouldn't be a priority.

That is unless Michael and Lucifer decide to bury the hatchet (and after being at odds for eons I doubt it).

Sam, also wouldn't be able to suffer the emotional scars of torturing others for decades, unless he is torturing Jake (and again that assumes the angels didn't keep their deal with him).

Lucifer was responsible for torturing humans into becoming the first demons. I think he could work up something to mess with the guy who screwed up all his plans (sending him packing back to the cage).

What I wish it was going to revolve around: Time travel back to the time of Samuel Colt (with the Winchesters; there's an awesome symmetry there). But I know that's not going to happen.

What I'm sure it is going to involve: Dean's deal with Death and the repercussions of Crowley's deal with Bobby all while both Heaven and Hell struggle to find a balance again.

I'm also fairly sure that Sam showed up courtesy of Dean. Whether it's Sam or Lucy-Sam is anyone's guess. But it's going to deal with Death. Guaranteed.

Of course he could and why Dean said his time would be worse then what he went through. But that was before Michael was put down there as well.

Would Michael after eons of wanting to confront Lucifer (even going so far as to manipulate the world to start the end of days), just decide to leave his brother alone, so Lucifer could spend some quality time with Sam. Or would Lucifer and Michael be stuck in this limbo where neither could kill each other just fighting it out?

Or the other option is that Michael would join in with Lucifer and torment and torture Sam. I personally don't see that, but it could happen.

Just pointing out that with the addition of someone else to occupy Lucifer's time, Sam might not get the full attention that he would have gotten. It's still going to be bad. Just as the possibility of being not as bad as what Dean suffered.

I don't think Sam appearance would be due to Dean (except in relation to passing God's test, that both Sam and Dean did unlike Lucifer and Michael). Dean isn't acting in the episode (at least in my opinion) as if he had stacked the deck in any way.

Err, that was a typo. I meant Death instead of Dean. If we take Death at his word, he's every bit as powerful as God Himself, so it shouldn't be a problem for him to pluck Sam out of the pit if he truly wanted to. Especially since it sorta-kinda falls into his specific domain.
