Well either the pip system was useless as of 2364 or they still didn't know what rank he was in All Good Things. They should have retconned his ensign insignia.

All Good Things made quite a few screw-ups with the rank insignia. For example, in the flashback scenes, Data had a Lieutenant JG's insignia.
Meanwhile, another mistake in AGT regarding O'Brien was that they had him as the main bridge's conn officer, even though in Farpoint he was in fact the battle bridge's conn officer. The main bridge's conn officer was Lt. Torres, the guy who pointed his phaser at Q and got froze as a result. And no, I don't think he was related to B'Elanna Torres.
Whenever the decision was made that the Enterprise had a crew of over 400, I think the other ranks thing was inevitable. With that size crew, it resembles a naval vessel, not a 20th century space capsule.
To stick with officers only and have it make sense, they'd really have had to go with a much smaller crew (and probably therefore a smaller ship).
