Star Trek 365 Photo question

Hi All --
Can anyone identify the photo on page "005-006" of the Star Trek 365 book? For reference, it is the classic Star Trek logo carved out of rock -- I think I saw this years ago (maybe in Starlog?) but don't recall. Anyway, where did this come from? Who made it? It seems awfully involved and SO strange -- more in line with a show like Land of the Lost rather than a futuristic show like Trek. Interested to hear what people may know!

The photo credits in the back attribute that image to Devra Langsam, who was one of the earliest fanzine publishers and convention organizers. Presumably it's from her collection, which would include a lot of rare early stuff, I imagine. I'd guess that image was an early ad from NBC or something. Though I suppose it could've been created for a fanzine cover.

Thanks! That helps a lot. I tend to lean towards your theory that it is an NBC promo as it seems pretty professional.
