Request - Inlay Cards for SE Movie DVDs

I wonder if anyone can remember seeing online some fan-made inlay cards for the Special Edition DVDs covering Star Trek Movies (VI thru Nemesis).



Paramount stopped including them after The Final Frontier, but I recall seeing some specially created ones. They're not essential but finding and printing out the missing cards, would be great to complete a matching set.
