IIRC, Millennium Shock was an emergency commission when a scheduled book fell through/was delayed - hence it's by the range editor. Same thing happened later with The Shadow in the Glass, which was rushed written and yet one of the best reviewed books of the year (that one, btw, is also interesting in terms of the Doctor's - and the Brigadier's - dealings with Churchill. And Hitler).
