If you're talking about "Snow Storm" it can't be done without killing cops. There's no way you're getting out of the building without killing some of them that are blocking your exit. It just isn't possible.
Now once you get outside, that's different story, that's when you just want to run like hell and jump in the water and get the boat.
It took me about 5 tries but I just beat this mission last night.

There are a lot of missions where you won't be able to avoid the police.
A boat? I never see boats!
But do I have to KILL them?!
Yes, you will. As a matter of fact, for you to have gotten to that mission, you have to beat "Blow your cover" which involves killing cop after cop to get out of a building. So I'm not understanding the issue here. You've done it before, you'll do it again. It's just the way the game is designed.
When you come out of the one and only door of the old hospital you have to run like hell and veer to your right a little to go through the gap in the fence and run straight for the wall and over into the water. You should land pretty close to boat. Get in the boat and get out of "wanted" area ASAP. Look out for police boats.

I managed to get out of the old hospital without killing any cops. I unintentionally left some of the people you are meant to shoot alive and they got into a shooting fight with the cops. I just legged it out of the building.
I simply picked up the health pack and armor before picking up the package, then I simply sprinted past the cops and ran into the water outside of the building and jumped in the boat. It was a pretty easy mission thanks to the game guide. My buddy tried it quite a few times because he didn't know about the boat, it makes it a much more difficult mission.
Why don't you want to kill the cops? Your criminal and they're the law. It's nature for you to kill them. It's a fucking video game. Oh, you'll drive like a maniac and kill innocet pedestrians on your way over, jack cars, and get involved in heavy crime but don't you dare kill a fucking cop!
I don't understand that either. I mean, I'm the cop beat reporter at my university's newspaper, but I have no problem gunning them down in GTA4. It's a video game!

Indeed. I always thought it was odd, to put it kindly, that critics of the game are so focused on the cop killing. If I was the type to get morally outraged over a video game, I'd be thinking a lot more about the EMTs, firefighters, and various other characters that aren't trying to kill you.
Seriously. Killing computerized, pixelated cops in a video game is not going to put you on Santa's Naughty List. I had no problem gunning the cops down in that level, but I would never even consider shooting a cop in reality. It's called separating the real from the unreal.
