Spoilers Extragalactic roll-call

I don't know that the Douwd are necessarily extragalactic. Kevin says he's "walked this galaxy," but I can easily imagine (say) 20th century Flint saying he's "walked this Earth" without trying to imply he's not from it.
Actually he says, "I have lived in this galaxy for many thousands of years." To most ears, that means he lived in another galaxy prior.
 
For what it's worth, the Star Fleet Universe has a number of different "extra-galactic" categories.

Firstly, there are those which arrived in the Milky Way Galaxy from elsewhere. As mentioned earlier in the thread, this includes the SFU's take on the Tholians (who are said to be from the M81 Galaxy, which they had ruled for several centuries until they were overthrown and forced into exile) and the Andromedans (who are from M31, but who seemingly have no connection to the on-screen Kelvans).

Others include the Juggernauts (a series of AI-controlled robot starships), the Seltorians (insectoids from M81; they had been uplifted by the Tholians to serve as "enforcers", only to later revolt and replace the Tholians as rulers of that galaxy), the Iridani (an otter-like species native to a star cluster orbiting the Milky Way, who send numerous Quests into the Omega Octant of the Milky Way), the Bolosco (an amoeba-like species of traders who fled their home galaxy; a group of Bolosco settled in the Omega Octant), and the Zosmans (another set of extra-galactic arrivals, who have an ongoing rivalry bordering on hatred with the Bolosco - and who don't get along well with the Iridani either).

Secondly, there are other galaxies which have various star-faring species of their own. These include the Lesser Magellanic Cloud, which was used by the Andromedans as a springboard for their invasions of the Alpha and Omega Octants of the Milky Way; as well as the Triangulum Galaxy, which has a number of empires available for playtest in Star Fleet Battles. Plus, there are (or were) other space-faring species in M81, to include a set of High Pirate Bands which opposed ot the Tholians and the Seltorians.

I should also note that the Milky Way itself looks somewhat different in the SFU than it does in the post-1979 Franchise. Instead of quadrants, there are 24 sectors divided into five galactic octants, where "life as we know it" may be found. The Federation, Klingons, and several other empires are in the Alpha Octant; the Omega Octant (where the Bolosco and Zosmans fled to) lies beyond a mostly-inaccessible Void region.
